How we help

How We Build a Toledo Disability Claim

At reconsideration, the first appeal after an initial denial, we address the reasons SSA gave and add relevant missing evidence.

For an initial application, we help gather medical evidence and document your work history. At reconsideration, the first appeal after an initial denial, we address the reasons SSA gave and add relevant missing evidence. If the case reaches an administrative law judge hearing, we develop the record, prepare you for questions, and present the case. These steps connect the evidence to the issues being decided.

We handle SSDI cases for people who currently receive VA benefits, but we do not handle VA claims or appeals. We do not take disability cessation or continuing disability review cases, or new cases involving back-pay issues alone. We refer federal court appeals to a trusted affiliate rather than handling them in-house.

Local medical evidence

UTMC Records for a Toledo Disability Claim

If you receive care through The University of Toledo Medical Center (UTMC), do not assume a MyChart download contains every relevant record. UToledo's MyChart terms state that the information displayed is not the complete medical record. A portal can help you start a treatment list, but you should check which visits, reports, and tests are actually included.

UTMC's medical-record release instructions describe a hand-signed, dated authorization with specific information completed. Incomplete forms may delay processing. The instructions distinguish inpatient records, such as discharge summaries and procedure reports, from outpatient visits and testing, and provide separate instructions for substance-use treatment records. Use the current instructions for the records you need.

Make a separate entry for each hospital, clinic, specialist, or other treatment source. Record the dates involved and what is available or still missing. If you also received care elsewhere, include that source rather than treating one portal as the whole history. SSA asks for medical sources, treatment dates, and evidence already in your possession; it says not to delay applying because you lack documents.

Get organized

A Medical-Evidence Tracker for Toledo Applicants

Use one row per treatment source. Update it when records arrive, and keep copies of any requests. This gives you a concrete way to discuss gaps in the medical history.

Treatment source Information to record What to track
UTMC, if it treated you Visit or admission dates; requested notes, reports, and tests Portal records saved; authorization sent; records received or missing
Each other hospital or clinic Provider name, contact information, and treatment dates Records already available and requests still pending
Provider for new treatment or testing Date, treating source, and what changed Whether the new evidence has been identified for your claim

Keep a separate work-history list with job dates and the tasks you performed. Note changes such as reduced hours, added breaks, or stopping work, using your actual experience. SSA's application guidance lists the medical and work information it requests.

After a denial

Appealing a Toledo Disability Denial

Deadline

60days

After an initial SSDI or SSI denial, a written request for reconsideration is generally due within 60 days after you receive the notice. After an unfavorable reconsideration determination, a written request for an administrative law judge hearing is generally due within 60 days after receipt.

SSA presumes receipt five days after the notice date unless you show that it arrived later. These are stage-specific rules, not one deadline for every SSA letter. See SSA's SSDI reconsideration rule and SSI reconsideration rule.

If the applicable filing period has passed, ask SSA for an extension in writing and explain the reasons for the delay. SSA must find good cause; an extension is not automatic. Save the complete decision and any information showing when it arrived. Do not wait for a medical-record request to finish before addressing an appeal deadline.

Our Ohio disability guide provides statewide context. If your appeal reaches the hearing stage, our hearing-office guide explains office details. Follow your own SSA correspondence for case instructions.

Common questions

Toledo Questions About Evidence and Representation

Talk through your own claim with us.

(615) 255-4307

We respond within 3 business hours.

Will a UTMC MyChart printout be enough for my claim?

Do not assume it is complete. UToledo says MyChart does not display the full medical record. Compare the documents you have with your treatment dates and the reports relevant to your condition. A record request may be needed for information that is missing; a portal download alone does not establish that you meet SSA's disability standard.

Should I wait for every medical record before applying?

No. SSA says not to delay applying because you lack documents and will help obtain them. Identify your treating sources and provide evidence already in your possession. SSI applicants should also begin the process without waiting for every document; SSA's SSI document guidance explains this.

What should I write down about my past jobs?

Record when you held each job, the tasks you performed, and how the condition affected those tasks. Include actual changes in hours, duties, or attendance. SSA requests work information as part of the disability application. A job title by itself may not describe what the work required.

Can Downard work with me while I live in Toledo?

Yes. We represent adults with SSDI and SSI matters in Toledo and work with clients by phone or video. We are based in Lebanon, Tennessee, and do not have an Ohio office.

Can I ask for help after reconsideration is denied?

Yes. We handle administrative law judge hearing matters within our SSDI and SSI service scope. We help develop the medical record, prepare you for questions, and present the case. Read the filing instructions in the reconsideration decision and contact us promptly rather than waiting for more records.
